Hicks Construction Subcontractor Application Step 1

If you’re thinking about joining Hicks Construction Company as a subcontractor, please fill out and submit the form below. After you submit the form you will receive an email from Hicks Construction in which you will be prompted to send:

• A signed W-9 form providing the Federal Employer Identification Number (FEIN)
• if you are looking to be hired for work more then $300,000 please also include :
• Most recent 3 years of Workers Compensation Experience Modification Rates
• Signed letter from your Surety Agent, and related power of attorney, stating the Surety name, your Company’s per project and aggregate limits. This letter must be dated within the last 12 months.
• If your company has more than 10 employees, most recent 3 years’ OSHA 300 and 300A. Due to the personal information on these logs, please black out the employees’ names prior to submitting the documents.
• Most recent 3 years’ audited financial statements.

You should expect an email from us on how to send us your W-9 and if you are looking to be hired for work more then $300,000, the following must also be included :
